At Kissito Post Acute the Brian Center Alleghany, we combine a beautiful, state-of-the-art healing environment with a multidisciplinary medical treatment team to develop individualized care plans for each of our patients. These plans are based on proprietary Collaborative Care Pathways to help guide treatment and ensure our patients are receiving the best possible care

The Kissito Post Acute Return to Home Program:

Kissito Healthcare has spent the last 12-18 months developing a patient centered post acute model of care. This patient-centered approach emphasizes collaboration, coordination and partnership among patients, families, hospital caregivers and post acute healthcare teams to produce smoother care transitions, improved outcomes and reduced costs.

Kissito Healthcare’s Assisted living program offers a housing alternative for older adults who may need help with dressing, bathing, eating, and toileting, but do not require the intensive medical and nursing care provided in nursing homes.

Located on the campus of our post acute and skilled nursing units, and adjacent to the region’s acute care hospital campus; you can be assured your loved one is going to be in the safest environment should they need more extensive services beyond our assisted living facility.
